Transitional Care Support
Our primary objective is to guarantee that patients receive consistent and coordinated care, characterized by seamless communication throughout their comprehensive healthcare experience. The Affiliated Hospitalists team is dedicated to assisting patients with the transition from post-acute care facilities. In collaboration with families, we provide resources to facilitate the coordination of services and support patients’ seamless transition, thereby minimizing the likelihood of subsequent hospital or emergency room admissions.
